2009 Tournament Registration**OPEN**
Registration Requirements:
A Medical Release Form signed by a parent or legal guardian is required for each player. Players without a release form will not be eligible to participate. Medical release forms for each player must be kept with the team at all times during the tournament.
A Team Release Form is also required for each team. The Team Release form must be submitted to the tournament committee.
All applications and checks (payable to Gates Youth Soccer) must be submitted to the address below by June 18th. Applications and fees cannot be accepted after this date.
Registration fees are:
- US$350.00 teams U9-U10
- US$395.00 teams U11-U19
In addition all participants must abide by the following tournament rules:
